Patient Information
First Visit
Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Thereafter, your appointment for treatment can be made.
You may do your paperwork online by clicking the Patient Registration tab at the top of the screen. Please make sure you fill out all the information on the Patient Registration Form and the Health History Form. If you were given a referral letter when seen by your general dentist, please bring that with you. If you would like to do your paper work in office rather than online, please come 20 minutes early to your scheduled appointment time. If you would like us to send you the paper work in the mail please call us at 541-617-3993.
IMPORTANT: All patients under the age of 18 years of age must be accompanied by a parent or guardian at the consultation visit.
A pre-operative consultation and physical examination is mandatory for patients undergoing IV anesthesia for surgery.
Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.)
X-Rays
If your dentist or physician has taken x-rays, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. On occasion additional films are necessary, and can be taken at our facility. All x-rays taken at our office are digital and can be e-mailed back to your general dentist if needed.
