First Visit To Dr. Krueger
Your initial consultation allows us to meet you and fully understand your desires. Before your first visit, please fill out our online registration by going to the patient registration tab at the top of the page. If you would prefer to do your paperwork in the office, please arrive 20 minutes prior to your appointment.
During your first visit you will meet with Dr. Krueger and his team to discuss your treatment. Throughout this process you are encouraged to ask questions as we want you to be comfortable and understand your options. During this consultation, please inform our team if you have any medical conditions that may be of concern (ie: diabetes, high blood pressure, artificial heart valves and joints, and any medications. Once we have decided on a course of action specific to you, we will then make your first treatment appointment.
IMPORTANT: All patients under the age of 18 years of age must be accompanied by a parent or guardian at the consultation visit.
X-RAYS: If your dentist or physician has taken X-rays our office will request them on your behalf. There are times when additional films will be necessary and will be taken at our office.
